There is clear consensus that a robust primary care system capable of managing the health of populations is critical to the success of our health care system. Yet too few clinicians enter primary care practice, and those in practice often lack key competencies to effectively manage populations.

The goal of Accelerating Primary Care Transformation at Jefferson (JeffAPCT) is to measurably improve the health of populations served, create an exceptional experience of care, and decrease costs across our entire population, with a particular emphasis on improving outcomes and decreasing disparities for traditionally underserved segments of our population.

JeffAPCT will develop, implement, and evaluate new curriculum and practice transformation strategies across the full continuum of primary care education, from medical and physician assistant students through primary care residents, practitioners, and faculty. Novel multimedia learning technology will support implementation. JeffAPCT will create infrastructure to support enhanced recruitment of under‐represented minority primary care clinicians, and recruitment of primary care clinicians into rural and urban underserved practice sites.

Specific Objectives:

  1. To impact Triple Aim outcomes through improved/expanded primary care and population health curriculum across the continuum of primary care providers and trainees
  2. To create enhanced, sustainable models of primary care physician faculty development to support PCMH Transformation
  3. To create a new, sustainable model of faculty development for community‐based primary care preceptors (MD/DO, PA, NP, others).

For Preceptors

JeffAPCT supports preceptors in a variety of ways. Our team will support current primary care providers and practices to undertake the hard work of practice transformation through a robust series of Practice Learning Collaboratives. These collaboratives will give academic faculty in primary care medicine and physician assistant studies as well as community primary care preceptors new skills in primary care, population health, quality and safety to support their own professional practice and to facilitate effective education of the next generation of primary care providers. In addition, an annual preceptor workshop will be held each spring to offer the chance to network, highlight best practices in precepting, and share resources.

In a move away from the traditional precepting model, we’re exploring how learners—including medical, PA, and NP students and internal medicine/family medicine residents—could to complete QI projects based on their preceptor’s needs and priorities. Students will also select projects focused on population health, quality and safety, or patient-centered medical home (PCMH) transformation activities.

For Residents

JeffAPCT will prepare primary care residents with the new tools of population health and PCMH transformation by building upon the already strong Family Medicine residency program and supporting the new Primary Care Internal Medicine residency track. Residents in these programs will be offered access to an online resource library for population health, PCMH, and quality and safety curricula, as well as additional support for leading interprofessional QI project teams.

For Students

JeffAPCT will expand population health, quality and safety, and PCMH experiential and didactic curriculum for medical and PA students. We are formalizing elective and selective opportunities for students to participate as members of PCMH teams serving underserved populations throughout Philadelphia, including the Stephen Klein Wellness Center, Philadelphia City Health Clinics, Jefferson Center for Refugee Health, Jefferson Geriatrics primary care, and Pathways to Housing.
