Representative - a position created at the suggestion of our PFAC

The Patient and Family Advisory Council (PFAC) partners patients and families with health professionals as members of the healthcare team to provide critical feedback on how to improve the patient experience and deliver more effective care. Patients and their families are recruited to serve on Jefferson’s Department of Family & Community Medicine’s PFAC to ensure that their point of view, perspective, and experience are not only heard, but integrated into quality improvements to ensure high-quality, patient-centered care. 
 

PFAC Accomplishments

We have implemented several changes since the launch of our first PFAC meeting in September, 2014. These changes have directly improved the patient experience. New signage on registration desks is helping patients navigate where they need to go. Rejuvenation of an outdated website is reintroducing our patients to their healthcare team. A new and improved patient brochure has been introduced to answer all questions patients may have and to let them know when their caregiver is seeing patients. Currently, one of our PFAC members is translating the brochure into Spanish; addressing language barriers is always on the agenda of every PFAC meeting. We now have a greeter at the front entrance of the practice to welcome patients upon their arrival and to address any questions and concerns they may have. On the horizon are more education materials and monitors providing appropriate medical education. We are also planning our first annual patient appreciation week in the spring.

We are currently recruiting interested patients and their families to become members of the Patient and Families Advisory Council.

We are seeking patients who can help transform our practice to ensure the optimum patient experience with delivery of the highest quality of care.

We want you and your family to participate in the process of care as full members of the healthcare team.

We ask for you to join us on the Jefferson Campus to attend two-hour breakfast meetings every other month from 8 -10 a.m.
